Please be aware that when scheduling an appointment for consultation, there are charges for your specialist evaluation and for any x-rays / photo documentation that may need to be taken. Please contact our office for these charge amounts.

Our participation in any insurance plan is subject to change. If you do not see your dental plan listed, please contact our office to inquire about whether we can help you obtain payment from your insurance.
